What to Expect?

Our Evidence-Based Lifestyle Program is led by a team of advanced registered dietitian nutritionists who prioritize whole food, plant-based eating. This 12-month program is conducted in a group setting, offering engaging group discussions and peer support. It is designed to incorporate the best practices of optimal nutrition, exercise, and behavioral psychology to support your whole-person health.

Our Evidence-Based Lifestyle Program is designed for patients with a variety of chronic conditions including:

  • Type 2 Diabetes

  • Chronic Kidney Disease

  • Hypertension

  • Excessive body weight, and

  • Metabolic disease

Our classes are once or twice a week over telehealth.  Then twice a month after the first 3 months. 

  • Classes start every quarter.  Call us to get exact date the next one starts.

  • We currently are using the Full Plate Living and Complete Health Improvement curriculums.
